Due to the nature of Helen House and who we serve, your signature is required to show your consent to the Confidentiality Agreement and Photographic and Liability Release below.
Helen House Confidentiality and Release Agreement
Helen House adheres to a strict policy of confidentiality. No employee, volunteer, board member, agent doing business or visitor with Helen House shall disclose confidential information gained by reason of her/his position, business or purpose with Helen House. Such confidential information shall also not be used for personal gain or benefit. .
Photographic Release
Volunteer does hereby grant and convey unto Helen House all right, title, and interest in any and all photographic images and video or audio recordings made by Helen House during the volunteer’s activities with Helen House, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
Liability Release
I acknowledge that participation in this work involves some risk of physical injury. I have been informed of and agree to expressly accept and assume any and all risks of injury or sickness arising from such participation. In consideration of participating in the program, I hereby agree to waive, release and forever discharge Helen House and its volunteers from any and all liabilities from participation in this activity.
Discrimination Policy: Volunteers of Helen House shall be selected entirely on a nondiscriminatory basis with respect to age, sex, race, religion, national origin, and sexual orientation or gender identity. It is the policy of Helen House not to discriminate on the basis of race, creed, ancestry, marital status, gender, sexual orientation, age, ability, veteran’s statue, political service or affiliation, color, religion or national origin. I agree to adhere to this policy
I hereby certify that all information contained in this application is true and correct. I understand that any misrepresentations, falsification, or consequential omission of information may render this application void and end my affiliation with the organization as a volunteer. I authorize the individuals and institutions named above to five information regarding my employment, character, and qualification, hereby releasing them from all liability for issuing such information.
